What documents will I need to complete the application?
For the application to be considered fully complete, you will need the following documentation:
1. Completed Gradelink application.
2. Signed Preschool Potty-Training Policy (when applicable)
3. Signed Parent/Student Handbook Acknowledgement
4. Signed and initialed Statement of Faith
5. Signed uniform contract.
6. Signed and initialed tuition contract
7. Signed liability, conflict, and arbitration contract.
8. Signed statement of cooperation
9. Copy of Birth Certificate
10. Health Inventory Form (Signed and Stamped by a Doctor)
11. Lead testing certificate filled out by a Doctor.
12. Signed Emergency Form
13. Student's transcript/record of achievement (at least two years of information: report cards, standardized test results and other professional records, such as testing, counseling, special education (i.e. Individual Education Plan, etc.).
What other steps must be taken to complete the registration process?
This varies widely depending upon how long it takes for you to finish the application form and submit all needed documents. You should allow up to six weeks although we can speed this up if you get everything to us quickly. Once we have all required information from you, application decisions are made very soon after that.

- You will be required to pay the $150.00 enrollment fee, this includes your FACTS set up fee. Please do the following:
- Set up a FACTS management account (please click on FACTS logo at the top of this page). Once this is set up, someone from the office will call you regarding your enrollment fee.
